Pereiaslav CNAP reiterates that since December 1, 2021, passport registration stamps were abolished and an extract from the territorial community register is now the only universal confirmation of a person’s registered address. The extract can show current and previous registration periods and carries a unique number and QR code to verify authenticity.
You can get the extract online via the Diia portal or mobile app, or in person at CNAP or at the relevant starosta office. Electronic and paper extracts have equal legal force. Teenagers aged 14 and over with a passport can generate their own extract in Diia; for children under 14, parents request it using the child’s birth certificate. A representative may apply with a notarized power of attorney.
For an in‑person request bring an identity document, tax identification number (RNOCPP), and, if relevant, a document proving ownership of the dwelling. Legally the extract is indefinite, but some institutions ask for a copy issued within one to three months. If the register lacks your data (for example, in certain temporarily occupied areas), CNAP advises updating records via Diia or by visiting the CNAP or starostat with original documents.
